How do I know if I or my individual in services qualify for day program services?

A Place to Belong serves adults with developmental disabilities, aged 18-90+. Currently, our services do not extend to minors. We embrace diversity and understand that each person has unique needs. Thus, transparency regarding the required support and care an individual needs is crucial when applying to our day program. This ensures we can collaborate effectively to assess whether our services align with the needs of the prospective participant.

Which waivers do you work with?

A Place to Belong is in Good Standing with the State of Colorado. We are currently working with Medicaid waivers specific to adults with developmental disabilities such as: Developmental Disabilities (DD), Non-Medical Transportation Services, and Non-Residential Supported Living Services (SLS) in the Douglas, Arapahoe, Adams, Denver, Jefferson, Broomfield and Boulder Counties.

How do I enroll myself or my individual in services?

The simplest method is to visit our contact page to initiate the process. You'll find a form for general inquiries, with optional fields to elaborate on your specific needs and the type of support you're seeking. These are submitted directly to our email. Additionally, we accept RFPs (Requests for Proposal) through case management and may contact you if we determine that our services can meet your or your individual's needs. Furthermore, our email address is available on the contact page, allowing you to reach out to us directly.

How long does the application process take?

The application process involves both you and case management. Initially, we arrange a meet and greet to discuss the day program, our operations, and the expectations from both parties. If desired, we offer free trials. The final step is to coordinate with your or your individual's case manager/resource coordinator to gather the required information and arrange billing on their individual service plans. Once this is finalized, we can proceed.

Will individuals be with the same groups every day?

Most often, groups will meet up in the community accompanied by other staff members. This practice ensures that groups can interact with their friends and peers while also enhancing safety through the presence of multiple staff to address any situations. Additionally, we enjoy coordinating with other agencies during outings to provide mutual support. Such interactions expand the horizons of the individuals we serve, exposing them to a diverse range of people, community members, and peers.

What sort of trainings do Staff undergo?

A Place to Belong ensures proper training for all staff, including but not limited to; QMAP (med administration), CPR, First Aid, Toileting Changes, Heimlich Maneuver, Flu Prevention, Transportation and much more.

Will I need to provide transportation?

Currently, A Place to Belong offers transportation services. Instead of operating from a fixed location, we collect individuals from their homes each morning and return them at day's end. This approach maximizes community engagement time and enhances the quality of activities.

What is the staff to individual ratio?

The staff-to-individual ratio varies daily, depending on attendance. In compliance with Colorado state regulations, a single staff member may be responsible for 10 individuals at most. We strive to avoid this scenario, and groups typically join another staff member during community outings.
